To complete the Wish List Request Form you must have Adobe Acrobat Reader installed on your computer. Acrobat Reader is an industry standard software that allows people to download and print documents over the Internet. If you don't have the Adobe Acrobat Reader, you can download a free copy at adobe.com.

Submitting the Form
Submitting the form using the Submit Via Email button (Option One) is our preferred method of submission as it greatly reduces the time and resources spent to construct the 2007/2008 Wish List.

Option One
Submit Form Using Submit Via Email button

• Complete Form
• Click on Submit Via Email button
• Select Your appropriate Email Client and Click OK

• Click on Send Data File button

• Send email. Note: Please do not change the data file name.

Option Two
Submit Form as Email Attachment

• Complete Form
• Click on File > Save As
• Save Form to your hard drive as agencyname_wishlist08.pdf. Remember to save it in a place you will remember.
• Attach agencyname_wishlist08.pdf to an email and send to maria.williams@uwsem.org, subject line Wish List 2007/2008 Submissions. Include the name of your agency in the body of the email.

Option Three
Submit Form Via Fax

• Complete Form
• Click on Print button
• Fax your completed form to United Way 2-1-1, Attn. Maria Williams, at 313-226-9394
